MEDICAL CLAIMS:
1) In-patient with admission card facility:
Admission Notification:
It is a condition precedent to liability that prior to being admitted for surgery, medical or obstetric care that a pre-admission report from your treating doctor be forwarded to our Company, so that we may consider as to whether the surgical operation/delivery/medical treatment falls within the scope and limits of the Insurance Policy and give the necessary authorization to the clinic.
2) In-patient and out-patient claims:
The Insured Person should complete and sign a medical claim form (including Policyholder’s signature) and submit same to our company, together with original receipts or accounts and/or clinic bills and the treating doctor’s reports and prescriptions, within a period of three months as from the date services are rendered.
TRAVEL AND ASSISTANCE CLAIMS:
All Emergency Medical expenses will be approved by Europ Assistance Worldwide Services (South Africa) Limited at the time they are incurred.
No guarantee will be given in case of any suspicion of pre-existing illness condition.

In the event of a baggage, money or documents claim, you shall give immediate written notice:
- to the relevant carrier in the event of loss or damage in transit
- to the relevant police authority in the event of loss
- to Us in all instances
It shall be a condition precedent to liability that a copy of the relevant carrier or police report be submitted to Us when a claim is made.
The Insured Person should complete and sign a Travel Insurance claim form (including Policyholder’s signature) and submit same to our company, together with original receipts, treating doctor’s reports and prescriptions or the relevant carrier or police report not later than 30 days after conclusion of the journey.
